Last date to claim incentives: July 31, 2026

REDEMPTION

  1. Purchase a next generation Midmark Steam Sterilizer and donate or dispose of your old sterilizer
  2. Click here to redeem
  3. Upload the invoice for your new sterilizer purchase
  4. Complete the information required on the form
  5. Select “Submit”

All serial numbers of the newly purchased sterilizers must be submitted online. Upon each submission, a confirmation email will be sent for your records. Please keep the confirmation until incentives are received. Redemptions must be submitted online by July 31, 2026.

The invoice must be dated between January 5, 2026 and June 30, 2026. Incentives will not be processed until the invoice is received. Midmark reserves the right to cancel or alter the terms of this offer at any time.

This offer is available for dental practices only as sold by Midmark authorized dental distributors in the US. This offer constitutes a discount or reduction in price on the purchased product(s). Buyer/Lessee/Lessor is obligated to: (i) fully and accurately disclose the amount of this discount in Buyer/Lessee/Lessor’s cost reports or claims for reimbursement to Medicare, Medicaid or other US federal healthcare program that requires such disclosure; and (ii) provide documentation of the discount to representatives of the US Secretary of Department of Health and Human Services and state agencies upon request.
