Last Date to Ship from Midmark to the Dealer: December 4, 2026
Last Date for Dealers to Invoice End-User: January 29, 2027
Dealer invoice to the Customer must be dated: March 2, 2026 - January 29, 2027
Last date to claim incentives: February 26, 2027

REDEMPTION INSTRUCTIONS

  1. Fill out the trade eligibility form below and follow on-page prompts
  2. Purchase an eligible product
  3. Once prior steps have been completed, fill out the rebate redemption form
    1. Upload invoice*
    2. Upload World Vision® confirmation email
    3. Complete the information required on the form
    4. Select “Submit”

Eligibility is subject to World Vision's needs and ability to accept trade-ins at the time of order or purchase. Upon each rebate redemption form submission, a confirmation email will be sent for your records. Please keep the confirmation until incentives are received. Redemptions must be submitted online by February 26, 2027.

* Midmark must receive a copy of the invoice (stating the physician’s name or facility name and date purchased). The invoice must be dated March 2, 2026 – January 29, 2027. Incentives will not be processed until the invoice is received.

 

This offer constitutes a discount or reduction in price on the purchased product(s). Buyer/Lessee/Lessor is obligated to: (i) fully and accurately disclose the amount of this discount in Buyer/Lessee/Lessor’s cost reports or claims for reimbursement to Medicare, Medicaid or other US federal healthcare program that requires such disclosure; and (ii) provide documentation of the discount to representatives of the US Secretary of the Department of Health and Human Services and state agencies upon request. Buyer/ Lessee/Lessor may request additional information from Midmark if necessary in order to meet its reporting or disclosure obligations.

This offer is valid on eligible products purchased by a physician or medical facility through an authorized Midmark dealer. This offer may not be combined with any other offers, promotions or contract pricing. Midmark reserves the right to cancel or alter the terms of this offer at any time.
