TRADE-IN to Trade-UP PROMOTION

Eligible Products

For each eligible product purchased during the promotion, accompanied by eligible trade, you will receive the rebate defined below.

TIMING

Dealer order period: March 2, 2026 - October 30, 2026
Last Date to Ship from Midmark to the Dealer: December 4, 2026
Last Date for Dealers to Invoice End-User: January 29, 2027
Dealer invoice to the Customer must be dated: March 2, 2026 - January 29, 2027
Last date to claim incentives: February 26, 2027

REDEMPTION INSTRUCTIONS

1. Fill out the trade eligibility form below and follow on-page prompts
2. Purchase an eligible product
3. Once prior steps have been completed, fill out the rebate redemption form
   1. Upload invoice*
   2. Enter your Trade Eligibility Form Confirmation ID (located at the top of your confirmation email)
   3. Complete the information required on the form
   4. Select “Submit”

Upon each rebate redemption form submission, a confirmation email will be sent for your records. Please keep the confirmation until incentives are received. Redemptions must be submitted online by February 26, 2027.

* Midmark must receive a copy of the invoice (stating the physician’s name or facility name and date purchased). The invoice must be dated March 2, 2026 – January 29, 2027. Incentives will not be processed until the invoice is received.
